"This is a wonderful day for me, my family and other loved ones," Perisic said on the club website.
"We had a great time here last season.
It was a tough year, of course, with a lot of goals and assists for me personally.
"But winning the national title was ultimately the absolute highlight for all of us.
Our fans were also fantastic, and I want to thank them for that.
But just as I expect from the players, I also expect our supporters to take another step forward together next season.
"The 36-year-old former World Cup finalist joined PSV in September 2024 and went on to win the Dutch Eredivisie title after a spectacular comeback in the final weeks.
Perisic had a significant impact on the comeback, scoring seven goals and providing three assists in the final eight league games.
Football director Earnest Stewart called Perisic an example to other players.
"Ivan is a wonderful example of what it takes to reach the very top and stay there.
Last season, he did a great job of bringing other players along, and that helps us enormously.
We are all very happy that Ivan is staying at PSV.
